A systems-based learning module using the BP Texas City explosion to explore how process safety information, management decisions, human performance and operational conditions interacted over time.
The focus is not simply on what happened. Participants examine what the organisation could see, what remained outside the management picture, how different parts of the Process Safety Management System connected, and what similar patterns might look like in their own organisation
About this module
BP Texas City explores how cost pressure, reduced staffing, fatigue, deteriorating working conditions, procedural deviations and degraded safeguards combined across the refinery.
It examines why the organisation’s safety information and process safety assurance failed to reveal the increasing vulnerability of the system.

Applying the learning
Participants consider what the combined warning signs meant and where comparable conditions could remain disconnected or unseen within their own organisation.
