A systems-based learning module using the Chernobyl accident to explore how hidden design vulnerabilities, trapped learning, hierarchy and changing operating conditions interacted over time.
The focus is not simply on the explosion. Participants examine what the organisation understood, what did not reach operations, when the test conditions changed, and why the point to stop was missed.
About this module
Chernobyl explores how technical design, secrecy, pressure to continue, uneven experience and the loss of operating margin combined within a high-hazard system.
It examines how a technically reasonable test became unsafe once the original operating conditions had changed and control had already been consumed.

Applying the learning
Participants consider where technically valid work may continue after conditions have changed, how known weaknesses reach operations, and what similar patterns might look like in their own organisation.
